The process is simple. Download and fill out the appropriate order form for your document by

to the proper location for retrieval. You have the option of Urgent Delivery or

A birth certificate is used for many purposes such as legal identity, proof of
age, sex or nationality, traveling abroad or applying for a passport, applying
for health services, a social insurance number or a driver’s license, child
enrolment in school, settling an estate, receiving old age pension.
The small certificate contains the individual's name, birth date, birth place,
gender, registration number, and registration date. The size is 9.5 x 6.4 cm
(3.75 x 2.5 in).
The large certificate contains the individual's name, birth date, birthplace,
gender, parents’ names, parents’ birthplaces, registration number, and
registration date. The size is 21.6 cm x 17.8 cm (8.5 x 7 in).
You would need a large certificate if:
- You are moving to another country
- You are an executor for a foreign estate
- You are adopting a child abroad
- You are filling out certain citizenship or immigration documents
Urgent Service includes the following:
- Review of your completed application, submission to the Office of the Registrar
General
- Follow up and tracing of application, delivery of the completed certificate
- Processed by the Government within 48 hours
- This time frame is only an estimate. It is NOT guaranteed.
- In all cases, your birth certificate will be sent by Express Post/Courier to you
as soon as the province has completed processing.
Regular Service includes the following:
- Review of your completed application, submission to the Office of the Registrar
General
- Follow up and tracing of application, delivery of the completed certificate
- Please allow at least 10-15 days for processing by the Government of Ontario.
- This time frame is only an estimate. It is NOT guaranteed.
- In all cases, your birth certificate will be sent by Express Post/Courier to you
as soon as the province has completed processing.
