Birth, Death and Marriage Certificates

Quebec

Immigroup can acquire the following certificates from the province of Quebec:

• Birth Certificates
• Abridged Birth Certificates
• Copy of an Act of Civil Status

Important Information:

• Anyone who has not received a Quebec birth certificate after 1994 requires a new birth certificate issued by the Province of Quebec. A baptismal certificate is NO LONGER VALID as a birth certificate.

• Delayed Statement of Live Birth
  (For people who have not registered their baby within 12 months)
• Amendment to the statement of birth
  (Father's name not on the certificate or mistake in information on the certificate)

Accelerated Service includes the following:

• Choose this option to receive your certificate within the province's accelerated delivery time frame.
• Once we have received your completed application, we will process it for you using the standard government procedure.
• In all cases, your birth certificate will be sent by Express Post/Courier to you as soon as the province has completed processing.

• Legible photocopy of either driver's licence, health card, other valid government-issued identification OR a valid Canadian passport OR a baptismal certificate.
• A copy of any bill (cable, phone, bank statement) showing your name and address.
