1. Download, print out and fill in a SIN Card application form and authorization from here. Please fill out and sign all forms, including the order form. It is absolutely imperative that a signed and dated Authorization Letter accompanies your application in order for Immigroup to submit your SIN request on your behalf. A letter template is included with the document package available from this website.
2. Attach the following original documents: a primary identity document, which proves your status in Canada. It could be any of the following: Canadian Birth Certificate, Citizenship Card, Record of Landing IMM 1000, Permanent Resident Card, Work Permit, or Study Permit. If you do not have any of the above documents because it has been lost, stolen, or destroyed, just click on the appropriate link above for details on how we can assist you in obtaining a replacement document.
3. Supporting documents if you intend to add a name on your SIN card, which does not appear on you primary ID document. It is also required when there is a name change due to a marriage.
4. If you are a parent or legal guardian signing an application for a Social Insurance Number on behalf of a minor, you must also provide your own identity document, i.e. a provincial birth certificate, permanent resident card, citizenship card, work or study permit, etc. On a separate sheet of paper, the parent or legal guardian must also provide their social insurance number.
