Sponsoring a Family Class Member:

Application Package

This application is for citizens and permanent residents of Canada who are sponsoring a spouse, common-law or conjugal partner, or dependent children.

The application is divided into three sections:

• Section 1
  - To be filled out by the sponsor in Canada
• Section 2
  - To be filled out by the person being sponsored
• Section 3
  - The person being sponsored must fill out forms and attach supporting documents specific to their region. The guides are region-specific. Following the link, please find the country from which you are sponsoring your spouse. The region-specific Document Checklist is available to download on this page.

You must send all completed forms from part 1, 2, and 3 together, at the same time to:

You must send all application forms (sponsorship and immigration applications) together. If you do not, or if these forms are not completed correctly, Citizenship and Immigration Canada will not process them. The applications will be returned to you.

Section 1

You must complete all of the forms that apply to you:

1. Application to Sponsor and Undertaking
   - This application requires generic information about the sponsor and the person being sponsored.
2. Sponsorship Agreement
   - This is a contract between the sponsor and the sponsored person guaranteeing that both parties will fulfill their obligations to each other.
3. Sponsorship Evaluation
   - The sponsor must list their financial situation on this form.
4. Statutory Declaration of Common-law Union
   - only complete this form if you sponsoring your common-law partner.
5. Sponsor Questionnaire
   - This form is a test of your eligibility to sponsor your loved one.
6. Use of a Representative
   - Only complete this form if you are using a representative to complete your application.
7. Document Checklist
   - This is a list of all supporting documents you must attach with your application.
8. Medical Condition Statement
   - Only complete this form if you intend to adopt an orphan from abroad.

Section 2

You must complete all of the forms that apply to you:

1. Application for Permanent Residence
   - This application asks for generic information about the sponsored person and their family members (if any).
2. Schedule 1 Background/Declaration
   - This application asks for detailed information about the sponsored person and a declaration that all information provided is accurate and truthful.
3. Additional Family Information
   - This questionnaire asks for details about the sponsored person’s extended family.
4. Sponsored Spouse/Partner Questionnaire
   - This form asks for details of the relationship between the sponsor and the sponsored person.
5. Use of  Representative
   - Only complete this form if you are using a representative to complete your application.
