I received my 3rd GCMS notes and found out that my DOB has been changed and my place of birth excluded. When I try to fill a webform, it says the informations don't match. Nor filing with the correct or the wrong DOB. On the notes, I could see when the change was made, but there's no explanation why.

Has this happened to anyone? What to do?
None of my documents has this mistake, and currently, this is my status:
ASSESSMENT
Eligibility: Passed
Security:
HIRV:
Criminality: Passed
Org Crime:
Medical: Passed
Misrepresentation:
Info Sharing: Complete
Other Reqs: Cancelled
Final:
