I am a Canadian citizen and we have applied for spouse inland sponsorship for my wife. We applied Online.

She is currently on a visitor visa in Canada which will expire in a couple of weeks.

At the time of submitting the application we also payed for work permit, but there were no instruction for applying for it.

My question is, can she apply for the work permit now or does she have to wait for approval in principle? If so, can it be done online?

She has gotten a request for the medical exam.

First, you need to extend her visitor visa (ASAP). Second, you can apply for an open work permit. This typically takes six months, at least after you have filed for the sponsorship. The important part is to keep her status while the applications are being processed.
