Verification of status - not sure what to put in fields

Hi - what do I put in fields that I don't have the answer to? Filling out 'verification of status' and I don't know the 'client id number' - do I just leave it blank?

This is for my deceased grandmother.

Put "not available" or the shortened form "n/a". Also write a Letter of Explanation, explaining in clear concise language why you don't have this information on behalf of your deceased grandmother. Go to the following link for our page on how to write one:
